Testing Forms
- Please fill out the GPAL Test Request Form and include it with your sample(s)
- To prevent delays in testing, please ensure the form is filled out completely. All information you wish to see on your report should be noted on this form as it is considered your contract with GPAL.
- Please double-bag the sample(s) to prevent spillage and co-mingling.
- If sending grain from outside the US, please note our USDA Import Permit #PCIP-19-00030 on your documentation to facilitate clearance through Customs
